Key Responsibilities:
· Conduct Care Act assessments and reviews
· Develop and implement personalised care and support plans
· Carry out safeguarding investigations under Section 42
· Work with adults with learning disabilities, mental health needs, physical disabilities, or older adults
· Liaise with families, carers, and multidisciplinary teams
· Ensure compliance with legal and ethical frameworks
What Were Looking For:
· Qualified Social Worker (Degree or equivalent)
· Registration with Social Work England
· Minimum 1 year of post-qualifying experience in Adult Services
· A strong understanding of relevant legislation: Care Act, Mental Capacity Act, etc.
